Terms Open account terms will be extended to all tax supported organizations, state, local, and federal government agencies. All other customers will be granted open account status upon credit approval. Call or fax FHM with request for credit application. OPEN ACCOUNT TERMS: NET 30 DAYS Payment All payments must be made in US Dollars. Past due invoices subject to service charge of 1.5% per month (18% annually). There will be a $20.00 charge for all returned checks. All customers without open account status must enclose a certified check or money order with mail order for the entire amount plus shipping charges. NO C.O.D. ORDERS ACCEPTED. FHM welcomes VISA or MASTERCARD. Prices subject to change without prior notice. Shipping/Freight/Handling FHM reserves the right to determine the most efficient means of shipping. All orders shipped F.O.B. ORIGIN (Shipping Point). Shipping charges will be prepaid and added to the invoice on all open account purchases. Whenever possible, merchandise will be shipped DHL, UPS or Parcel Post. Shipments leave FHM warehouse within 48 hours of order, subject to stock on hand. In 'out of stock' situations, items are automatically backordered and shipped as soon as possible. Minimum order is $50. Orders less than $50 will be charged a $7.50 Handling Charge. Warranties FHM offers no warrant of merchantability or fitness for a particular purpose or use. FHM products are not "consumer products" as that term is defined in public law 93-637, and FHM makes no warranties implied to "consumers" thereunder. FHM shall not be liable under any circumstances for consequential or incidental damages of any kind. Consult local medical authority for specific protocols relative to product use and patient care. Claims FHM carefully inspects, documents and weighs all orders prior to shipping. If there is a problem with damaged shipments, the customer should first contact the carrier. FHM will assist in any way in claims made against the carrier. In situations where the customer received a short shipment, FHM should be notified immediately. In the event that we have made an error, we will promptly make the adjustment at no charge to the customer.
All international orders will be shipped by UPS or DHL Courier, per Firehouse Medical’s discretion. FHM will contact you after receipt of your order to confirm your shipping method and delivery charges. *Minimum International Order = $150.00 U.S. Dollars* All international orders must be prepaid prior to shipment. If you have not paid for your order with Mastercard or Visa, and wish to pay by another method, we will accept payment by certified check or money order through the mail. Swift or wire transfers are also accepted. All payments must be made in U.S. Dollars. If you wish to wire funds for payment, contact us by: Email: sales@fhmed.com fax: 00 1 714 688 1577 and we will send you banking specifics for your transfer. A $20 wire fee will be added to your total for bank charges. Arrangements can also be made for payment by letter of credit with orders over $5,000.00 (USD). Please contact us for further information and bank specifics to set up payment by L.C. Firehouse Medical currently ships our innovative product line into over 50 countries worldwide. We contract with excellent freight forwarders that can deliver large shipments, or regular distributor shipments, to any airport or seaport in the world. Contact us by email or fax to request shipping by freight forwarder instead of courier service. In most instances, larger shipments will be cheaper to transport with freight forwarders.
